Create Gerber files for PCB from Schematic -- 2

I have a schematic that needs an update and converted to PCB and the output needs to be a gerber file suitable for sending out to be made. I am open to suggestions for both the power input (+12v) and the motor output regarding connectors. I did not build in (but needs it), protection from reverse power on the 12v.
