Custom Magento 2 form/block

We are looking to incorporate a custom Magento 2 “contact form/block” for the specific purpose of collecting URL’s to verify customer reviews. The form is to be presented to the customer, they copy paste URL’s into the text box’s and click submit to email them to us complete with notification to the customer that it has been submitted.
The form needs to integrate into our theme and the form needs to output an email to our sales@ email address for us to click the submitted links to verify the published review.
