I need some PDF knowlegeable

Hi,
I’m currently working on our company’s initiative of using a certain application as a project management site.
I realized that we have a problem in getting PDF fillable forms to work.
We have about 150 rows and 8 columns in excel that I need fillable forms for.
Otherwise the automatization of this process won’t work.
In the attachment is the placement of where I would need the fillable forms in.
