Below is an example of how I want the php form

Your name (person filling out the form)
Your email (person filling out the form emails address)
Their email address ( copy of the form output to be sent to this email address)
Text field box 1
Text field box 2
Text field box 3
Etc
Once the person fills out the form and hits submit I want the following
1) Human verification (Captcha)
2) A copy of the output gets sent to form fillers email address as well as an internal email address.
3) I want the form filler to click on a link in the email which then sends the form to (recipient)

So for example. I fill out the form. Input my email as sender, fill in your email as recipient and his submit
I verify captcha and wait for email to be sent to my email address. Once I receive email I click on the link inside body which than takes me to a page that sends out the output to your (recipient) email address.


This form will be hosted on a webhosting company which has php as well as email service.
