Simple WP Plugin

Hi there,
I just need a simple plugin which will do the following:
1)- Logged in users will Fill a Form in dashboard.
2)- The form result will be posted under Reports page.
3)- The form result will be altered based on the user input.
4)- Each user should only view their own form results and form should remain saved in database.
I need this done fairly quickly as it’s straight forward and all your queries will be answered since I’m online.
