Website code to get the user a custom external link

Need code for a form that you’d complete to get a custom link to an external website. Image attached would be ideally how it looks. I want to put this on my Shopify website to help people find reputable pool companies through an Australian pool company directory.

I tried using <iframe> but they have it blocked. I know very little about HTML and CSS. I’m very flexible with any minor things that can’t be done, let me know if this is the case. If you have a suggestion for a better way of doing this, I would also love to hear it. Basically it includes three things:
1. Box to enter postcode that will autofill with an Australian suburb
2. Dropdown menu for company type that has these four options: Builder, Fencing, Certification, Servicing/Retail
3. A search button that opens a new window to this website with this link and text that changes based on the options selected:

Input one is “2000” which is the postcode entered and input two is “1%2C4” which is one of four category options. Builder is “categories=1%2C4”, Fencing is “categories=3”, Certification is “categories=5%2C8”, Servicing/Retail is “categories=6%2C7”.

This is an example of how I’d want a user to interact with this:
Someone entered 2000 into postcode, it shows a dropdown of options for that postcode or autofills. The user then selects an option from the Company Type dropdown list, once they choose Builder. The search button is now clickable. When the user clicks it, a new window opens up with this link:
